Hydroxyprogesterone caproate is a synthetic progestin, also known as 17 hydroxyprogesterone caproate, is closer to progesterone in terms of structure and pharmacology than most other progestins, and is essentially a pure progestogen that is, a selective agonist of the progesterone receptor (PR) with minimal or no other hormonal activity.

Hydroxyprogesterone caproate is a synthetic progestin, also known as 17 hydroxyprogesterone caproate, is closer to progesterone in terms of structure and pharmacology than most other progestins, and is essentially a pure progestogen that is, a selective agonist of the progesterone receptor (PR) with minimal or no other hormonal activity.
However, Hydroxyprogesterone caproate has improved pharmacokinetics compared to progesterone, namely a much longer duration with intramuscular injection in oil solution. Hydroxyprogesterone caproate decreases contractility of the uterine smooth muscle. This effect contributes to prevention of preterm labor. Hydroxyprogesterone caproate is indicated to reduce the risk of preterm birth in women with a singleton pregnancy who have a history of singleton spontaneous preterm birth.
